Before creating an answer response group, all of the answer choices you want to include must already exist. See How to Create a New Answer Choice for more info.
On the Settings page, choose Response Choices and Answer Response Groups. Click the + sign to create a new answer response group.
Assign a name and description, and check the boxes next to the answer choices you want to include in the group. You should at least choose a favorable (value 1) response and an unfavorable (value 0) response. Then click Save.

You cannot create managed answer response groups.
