A questionnaire template is a list of questions used to build a survey or set of surveys. You can also think of it as a template for a certain type of assessment. Before you can create an assessment, you need to have at least one questionnaire template with the same target type as the assessment itself. For example, if you want to assess an application, then you need to have a questionnaire template that targets applications.
